SRN MOU

This Memorandum of Understanding (MOU) outlines the roles, responsibilities, and expectations for partners participating in the Smart Referral Network (SRN), including requirements for referrals, data sharing, and system use. It also establishes guidelines for privacy, security, and compliance to ensure client information is handled appropriately while supporting coordinated service delivery.

 

 

 

211 General SRN Intake Form

This form is used by partner agencies to apply for inclusion or submit updates to the 2-1-1 Monterey County and SRN databases. It collects standardized information about your organization, service locations, and programs to ensure your services are accurately represented and easily accessible for referrals through 2-1-1.

 

 

 

SRN Consent Form

This form is used by partner agencies to obtain permission for individuals to be referred through the SRN and to allow the secure sharing of personal information needed to connect them to services. It outlines what data may be shared, how it is protected, and the individual’s rights to access, limit, or revoke consent at any time.

SRN Intro & Overview Presentation

This presentation provides an introduction and overview of the SRN. It walks through key areas including access and permissions, infrastructure and security, referral and care management workflows, and training learning objectives to help users understand how to effectively use the platform while maintaining privacy and compliance standards.

Frequently Asked Questions

The SRN Dashboard helps agencies track their activity in the Smart Referral Network, including referrals, users, and partner engagement.

This shows how many users currently have active accounts in the system.

This is the total number of referrals your agency has ever been connected to. 

It includes referrals where your agency:

  • Sent (created) the referral 
  • Received the referral 
  • Is listed on the referral record 

Includes both past and current referrals (historical view).

This shows referrals your agency is currently working on. 

It includes referrals with statuses such as:

  • Waiting
  • Received
  • Enrolled
  • Some declined referrals 

Focuses on active/ongoing work.

SRN’s Total Referrals = all referrals over time (including closed ones) 

My Total Referrals = referrals currently being handled 

Simple way to think about it: 

SRN Total = history 

My Total = current workload

Some declined referrals are still included if they were received by your agency. Declined referrals your agency sent to others are not included.

These are referrals your agency has received from other agencies.

These are referrals your agency has sent to other agencies.

This shows which agencies have created or sent the most referrals.

This shows which agencies have responded to the most referrals.
